The Commission on Cancer, the American Cancer Society and the National Cancer Registrars Association are conducting nationally the Resources, Information, and Guidance Health Survey. The purpose of the study is to assess cancer patients' and their families' awareness, use of, and satisfaction with the resources and programs available in their communities. Texas data will be used by the Texas Cancer Council and the American Cancer Society, Texas Division to determine the types of services that are most needed by Texans with cancer and to establish a baseline for determining progress in developing services. The survey will be conducted through the Commission's Cancer Liaison Network, a grassroots network of physicians who treat patients with cancer. Local hospital cancer committees also will be involved; the hospitals' cancer registrars will identify potential survey recipients. The anonymous, completed survey forms will be collated and analyzed by staff at the American College of Surgeons. Data will be aggregated nationally, regionally, and at the state level. Hospitals will receive copies of the aggregate data for their patients who participated in the study, as well as the national results. The Texas Cancer Council will co-sponsor the Texas study. The injection zone for all three wells is the Lower Miocene Catahoula Formation between the approximate depths of 4, 800 and 5,450 feet. The maximum rate of injection for WDW-082 and WDW-083 is 260 gallons per minute (gpm) for each well and for WDW-149 is 520 gpm. The maximum annual volume of wastewater injected into WDW-082 and WDW-083 shall not exceed 136,656,000 gallons each and for WDW-149 shall not exceed 273,312, 000 gallons annually. The operating surface injection pressure shall not exceed 1,300 per square inch gage for each well. The wells are used to dispose of waste generated from the manufacture of organic chemicals at the La Porte, Texas plant site and to dispose of stormwater from a captured facility owned by NOLTEX, L.L.C. Waste disposal wells WDW-082, WDW-083, and WDW149 are located on company property approximately two miles north-northwest of La Porte. Water may be diverted from the FM 1566 crossing of an unnamed tributary of Tidwell Creek, approximately ten miles south of Greenville, Hunt County, Texas, Sabine River Basin. Provisionally-Issued Temporary permits to appropriate state water are issued for a period of not more than one year and authorize the use of not more than ten acre-feet of water. The Executive Director of the TNRCC has reviewed each application for the permits listed and determined that sufficient water is available at the proposed point of diversion to satisfy the requirements of the application as well as all existing water rights.
